Letter on 'Use of Form B-12 for Registration of Deaths in Hospices'
This letter issued by the Director, National Cancer Control Programme is addressed to all Chairpersons of the Hospices in Sri Lanka and it highlights the importance of using Form B-12 for registration of deaths, specially for the hospices that experience difficulties in the process.
